Registration Information
Your doctor has arranged for your admission to Wesley Medical Center. The information below will help ease the process. During admission you may be asked to provide information about yourself, as well as financial information.
Preregistration
If your doctor has given us advance notice of your inpatient or outpatient admission, someone from the Admissions Department may call you a few days before your scheduled visit to verify basic information, such as your address and next of kin, name of your employer, and the type of insurance that you carry. It would also be helpful to tell us if you have any special needs such a wheelchair or an interpreter. This information will speed up the registration process on the date of your arrival.
What to bring
Please remember to bring the following items with you:
- Your Social Security number
- All active insurance cards
- Picture ID (such as a driver's license or passport)
- Advance Directives (if you have these) - Living Will or Durable Power of Attorney.
- In some instances you might also need to bring a written physician order.
Documents you will be asked to sign
When registering, you may be asked to sign a Conditions of Admission (COA) or Consent for Outpatient Services (COS) form.
Medicare recipients
Medicare recipients will be asked basic questions to help us complete a Medicare Secondary Payor questionnaire and will be given a Medicare Rights Statement.
Champus/Tricare recipients
Champus/Tricare recipients will be asked to sign a Champus Rights Statement.
REGISTRATION AREAS
Wesley Medical Center has six separate registration areas. They include Main Admissions (Admissions North), Critical Care Building (Admissions Southeast), Midwest Center for Wound Healing and Hyperbaric Medicine, Wesley BirthCare Center, Ambulatory Surgery Unit, and Emergency Department.
Main Admissions (Admissions North)
Hours of operation are 5 a.m. - 7 p.m. daily. The Registration Desk is located on the first floor of the main entrance lobby at the corner of Hillside and Murdock (near the patients' parking garage). The phone number is (316) 962-2565. At 7 p.m. the Emergency Department registrars will assume the registration function for the outpatients scheduled between 7 p.m. - 9 p.m., and any direct admissions to the Emergency Department.
Critical Care Building (Admissions Southeast)
Hours of operation are Monday-Friday, 6:30 a.m. - 5:30 p.m. The Critical Care Building is located on the southeast corner of the hospital campus near the corner of Central and Rutan. Patients entering the building for outpatient tests and procedures including pediatric sedation, nuclear medicine, ultrasound, CT scans, vascular imaging, radiation therapy, pre-op assessment clinic, infusions and respiratory care should go to Admissions Southeast just inside the entrance. The phone number is (316) 962-9080. Any walk ins or patients that come in after 5:30 will be registered by the Main admissions staff until 7 p.m.
Mid-West Center for Wound Healing and Hyperbaric Medicine
Hours of operation are Monday-Friday, 8 a.m. - 4:30 p.m. (closed from noon - 1 p.m.). The Center for Wound Healing and Hyperbaric Medicine is on the first floor of the Critical Care Building, which is on the southeast corner of the hospital campus near the corner of Central and Rutan. Wesley BirthCare Center
Hours of operation are 24 hours a day, 7 days a week. Obstetric patients who are not high-risk should go directly to the reception desk in the BirthCare Center if they believe they are in labor. The BirthCare Center is located at 3222 E. Murdock at the corner of Hillside and Murdock. The phone number is (316) 962-3222.
Ambulatory Surgery Unit (ASU)
Hours of operation are Monday-Friday, 5 a.m. - 9 p.m. The Ambulatory Surgery Unit is located in Building 4 ("the Tower") on the second floor, inside the main hospital. The phone number for ASU is (316) 962-2351.
Emergency Department
Hours of operation are 24 hours a day, 7 days a week. The Emergency Department is located on the southwest corner of the hospital campus at Central and Hillside. All emergency patients entering Wesley Medical Center through the Emergency Department will be registered in this area. Any admissions occurring after Main Admissions is closed will also register through this area (except for patients going to the BirthCare Center).
